Сердечно-сосудистые эффекты финеренона
Финеренон является первым нестероидным антагонистом минералокортикоидных рецепторов (МКР), который имеет высокое сродство и селективность к МКР, а также значимые отличия от стероидных аналогов по физико-химическим, фармакодинамическим и фармакокинетическим свойствам. Ряд исследований подтверждает защитную роль финеренона благодаря противовоспалительному, антифиброзному и антиремоделирующему действию в сердечной и почечной тканях, что обеспечивает клиническую эффективность в снижении частоты сердечно-сосудистых исходов. В клинических исследованиях показано не только почечно-защитное действие финеренона, замедляющее прогрессирование хронических болезней почек (ХБП), что оценивается по совокупным почечным исходам/смерти от почечных причин, но и снижение частоты сердечно-сосудистых исходов (включая сердечно-сосудистую смертность, госпитализации по поводу сердечной недостаточности, нефатального инфаркта миокарда и нефатального инсульта). В специальных исследованиях показано благоприятное влияние финеренона на развитие впервые возникшей хронической сердечной недостаточности (ХСН) и исходы, связанные с ХСН, независимо от наличия ХСН в анамнезе. Кроме того, эффективность финеренона у пациентов с ХСН прослеживается по всему диапазону фракции выброса (ФВ), снижая риск сердечно-сосудистой смерти и частоты госпитализаций по поводу ХСН. Все имеющиеся данные указывают на то, что, будучи безопасным и хорошо переносимым, финеренон имеет высокий потенциал для предотвращения заболеваемости и смертности в широком диапазоне состояний кардио-ренально-метаболического синдрома.Леонова М.В.
